CSD may collect personally identifiable information, such as your e-mail address, name, physical address or telephone number (“Personal Information”). CSD may also collect other Personal Information that you voluntarily provide to CSD through responses to surveys, questionnaires, email requests, and similar communications.

CSD does not collect and store billing and credit card information. This information is handled by a third-party payment processor.

In addition, CSD secures the Personal Information you provide on computer servers in a controlled, secure environment, protected from unauthorized access, use or disclosure. You have the right to access, update and correct inaccuracies in your Personal Information within CSD custody and control, subject to certain exceptions prescribed by law. You may contact CSD to make a request about your Personal Information using the contact information set out in Section 7 “Contacting Us”.
You may be required to provide sufficient proof of your identity at this time to ensure the safety and security of the Personal Information CSD holds and that it is provided to individuals in accordance with this Policy. CSD reserves the right to decline to provide access to Personal Information where the information requested: would disclose Personal Information of another individual or of a deceased individual; is not readily retrievable and the burden or cost of providing would be disproportionate to the nature or value of the information; does not exist, is not held, or cannot be found; could reasonably result in serious emotional harm to the individual or another individual, or serious bodily harm to another individual; or may harm or interfere with law enforcement activities and other investigative or regulatory functions of a body authorized by statute to perform such functions.
